The driving examiner's ID number should be encoded into the driver's licence and readily accessible to authorities. In the really extreme cases of incompetence the examiner that actually passed the idiot should be called to account. (Similarly, parole board members should be called to account for any horrendous crimes committed by the parolees they approved to go free).

Not saying these bureaucrats should be held directly responsible for the crime/accident/driving offence committed, but neither should they not be held accountable. IMHO
